LifeCircles

LifeCircles is a Program Of All-Inclusive Care for the Elderly (PACE) that serves Muskegon, most of Ottawa and portions of Allegan Counties. The goal of LifeCircles is to provide support services to individuals 55 and older with chronic health issues to be able to age in their own community. Some of the services provided are day center attendance, home care, medical transportation to and from the day center and outside medical appointments, medical care, therapies and prescriptions. Because LifeCircles provides and is responsible for all of your care, you may be held financially responsible for any care you receive outside the program that is not approved by the PACE interdisciplinary team (except in emergency).
